The term was coined in 1967 by Italian sports journalist Gianni Brera. The teams are from the two biggest cities in Northern Italy. Both clubs are among the most successful football clubs in the world. The matchup is perhaps the most intense match in Italy between two teams from different cities, historically since the 1950s and 60s, and especially after the Calciopoli scandal which saw Juventus stripped of their league title from 2005–06 and given to Inter.

The derby has seen notable incidents, including a 1960–61 fixture where a field invasion led to a replay that Inter lost 9–1 after fielding youth players. In 1997–98, controversy over referee decisions sparked heated arguments in the Italian parliament. In 2009, there were fears of racial abuse toward Inter's Mario Balotelli, and the match featured multiple bookings and a red card. In the 2015–16 Coppa Italia semi-final, Juventus advanced after a penalty shoot-out.

• Juventus striker Omar Sívori scored six goals in the 9–1 win over Inter on 10 June 1961 and won the Ballon d'Or that year.
