The Alexandria Safe-Zone Breach refers to the critical security failures that allowed walkers to infiltrate the fortified walls of the community. While initially presented as an impenetrable sanctuary for survivors, these incidents shattered the illusion of safety and forced residents to confront the reality that no barrier could guarantee absolute protection against the dead.

Lore & Background
In the sixth season, while the group leads a massive herd away from a quarry, the damaged clock tower collapses and opens a breach in Alexandria's wall, and walkers invade. In the mid-season finale, "Start to Finish", Rick takes shelter in Jessie's house with Carl, Michonne, Gabriel, Deanna, Jessie and her sons; in "No Way Out" the Alexandrians fight the herd inside the walls. Later, in the episode "Stalker", the Whisperer Beta enters Alexandria at night through an underground tunnel.

In this world everyone is infected: anyone who dies reanimates as a walker, a fact Rick learns from Dr. Jenner at the CDC. A breach of the walls therefore threatens a community twice, from the herd outside and from its own dead.
