The realm of Sheogorath is split into two distinct sections: Mania, characterized by vivid colors and fantastical flora, and Dementia, filled with bogs and agonistically twisted trees. The history of this Prince is defined by a curse; Jyggalag, the Daedric Lord of Order, was once so powerful that other Princes feared him, cursing him with madness to transform him into Sheogorath. This transformation created an eternal cycle known as the Greymarch, where Jyggalag would return at the end of each era to destroy the Isles and then transform back into Sheogorath to start over. This reincarnation cycle meant that for eons, the Prince of Madness was doomed to relive his own destruction repeatedly until a specific event altered his fate.

In Their Own Story

The player enters the Shivering Isles just as the Greymarch begins, starting in an area called the Fringe before defeating the Gatekeeper to access the rest of the realm. After an audience with Sheogorath, who explains the threat, he tasks the player with stopping the event. However, these attempts ultimately fail. As the forces of Order invade the Court of Madness, Sheogorath transforms into his alter-ego, Jyggalag, and disappears. The player must then face Jyggalag directly in combat. Upon defeating him, Jyggalag reveals the truth about their shared history and thanks the player for breaking the cycle. In a historic turning point, he dubs the player the "new" Sheogorath, making them the Prince of Madness and ruler of the Shivering Isles.

To navigate Sheogorath's realm, players must understand that the main quest to stop the Greymarch can be ignored for as long as they wish. The expansion is open-ended, allowing players to define their character's skill set, race, and physical features just as in the base game of Oblivion. Progression relies on gaining experience points by practicing specific skills; for example, using archery increases archery experience rather than granting generic points. Enemies scale in difficulty alongside player strength. Once the expansion is installed, this content remains accessible to any character at any time. The realm features unique design elements credited to concept artist Adam Adamowicz, whose signature style defines the portal and landscape.
