Leptospirosis is a bloodstream infection triggered by bacteria from the genus *Leptospira*. It affects humans, dogs, rodents, and a wide range of other wild and domestic animals. The illness is a classic One Health concern because its spread relies on connections between people, animals, and the environment. Worldwide, about one million severe human cases occur each year, leading to roughly 58,900 deaths.
Symptoms can be absent, mild—such as headaches, muscle pain, and fever—or severe, including lung bleeding or meningitis. The acute, dangerous form is called Weil’s disease, marked by jaundice (yellowing of the skin and eyes), kidney failure, and bleeding. When bleeding happens in the lungs, it is known as severe pulmonary haemorrhage syndrome. Even with the best treatment, Weil’s disease kills more than 10% of those affected; when the lungs are involved, the death rate jumps to between 50% and 70%.
More than 20 serogroups and over 250 serovars of *Leptospira* cause human illness, with at least 13 distinct pathogenic species identified. Animals, especially rodents, spread the bacteria primarily through urine. People get infected when contaminated urine—or water or soil tainted by it—touches the eyes, mouth, nose, or broken skin. In developing countries, the disease mostly strikes pest-control workers, farmers, and low-income people in areas with poor sanitation. In developed nations, outbreaks often follow heavy rain, and risk is highest for pest controllers, sewage workers, and people doing outdoor activities in warm, wet places. Diagnosis usually involves testing for antibodies or finding bacterial DNA in the blood.
Prevention includes wearing protective gear when handling potentially infected animals, washing after contact, and reducing rodent populations near homes and workplaces. The antibiotic doxycycline is sometimes used for chemoprophylaxis in certain high-risk situations, though its effectiveness is not absolute and remains debated. Human vaccines have limited use, but vaccines for other animals are more common. Once infected, treatment uses antibiotics like doxycycline, penicillin, or ceftriaxone.
