Giardia duodenalis—also called *Giardia intestinalis* or *Giardia lamblia*—is a flagellated, parasitic protozoan that lives in the small intestine and causes the diarrheal illness giardiasis. It attaches to the intestinal lining using a ventral disc (also referred to as an adhesive disc or sucker) and reproduces by binary fission. The parasite is non-invasive: it stays within the small intestine’s lumen and does not spread to other parts of the digestive tract. It has two life stages: the active trophozoite and the dormant cyst. Through encystation, it transforms into a cyst that can survive outside a host. Trophozoites are anaerobic and absorb nutrients from the intestinal lumen. When stained, the organism’s shape resembles a classic smiley face.

People most often get infected by swallowing untreated drinking water—the most common transmission route—or by consuming food, soil, or sewage contaminated with human feces. This is especially common in many developing countries. Natural waters can also become contaminated in watersheds where intensive grazing occurs. Infections happen worldwide. Among children in day-care centers, hikers, and immunocompromised patients, *Giardia* is the most frequently identified intestinal parasite. In the United States, about 20,000 cases are reported each year.

Roughly half of infected individuals show no symptoms. When symptoms do appear, they typically start 1 to 2 weeks after infection. Common signs include abdominal pain, nausea, bloating, and large, watery, foul-smelling, greasy stools. Frequent loose stools often lead to dehydration. The parasite damages the intestinal epithelium, which directly impairs nutrient absorption. In severe cases, giardiasis can cause chronic diarrhea, chronic fatigue syndrome, and cognitive impairment in children.

**Life cycle**

*Giardia* has two distinct forms during its life cycle. Trophozoites are the replicative stage: pear-shaped, motile, flagellated cells that survive only in the host’s small intestine. They do not invade host cells but attach to the intestinal epithelium to establish infection. Cysts are the environmentally hardy stage that allows transmission between hosts.

Infection begins when a host ingests cysts. These pass through the stomach into the duodenum (the first part of the small intestine). Digestive enzymes and acidic pH trigger excystation, releasing trophozoites.
