The samples are vital for synthesizing a treatment against cardiovascular disease, leveraging the dinosaurs' naturally low incidence of heart issues. The island's ecosystem is hostile, populated by hybrid Mutadons, a hybrid raptor/pterosaur, and the elusive Aquilops, creating a lethal environment where survival depends on navigating both human treachery and prehistoric predators.

In Suriname, Zora Bennett assembles an expedition team with Duncan Kincaid to lead, alongside maritime pilot LeClerc, mercenary Nina, and security expert Bobby Atwater. They illegally travel to Île Saint-Hubert while a sailboat carrying the Delgado family—Reuben, his daughters Teresa and Isabella, and Teresa's boyfriend Xavier Dobbs—is capsized by a Mosasaurus attack. Despite Martin Krebs insisting on mission secrecy, the team rescues the family. The rescue turns deadly when Spinosaurus join the attack; Bobby Atwater is killed, and Nina dies after the ship runs aground. Martin prevents Teresa from radioing for help, allowing her to fall overboard before Xavier, Reuben, and Isabella jump in to save her, separating them from the team. Zora reveals a rescue helicopter will circle in 24 hours if there is radio silence. The expedition secures samples: a blood draw from the Mosasaurus using a modified dart gun, a tissue sample from a herd of Titanosaurus, and amniotic liquid from a Quetzalcoatlus egg retrieved by Zora and Henry after LeClerc is devoured. Meanwhile, the Delgados survive a Mutadon encounter and adopt an Aquilops named Dolores before finding an inflatable raft and escaping a Tyrannosaurus rex attack. Reunited at the InGen complex, Martin reveals his betrayal, threatening the group with a handgun before fleeing to the helipad. The rescue helicopter is destroyed by the Distortus rex. Survivors flee through tunnels where Isabella unlocks a gate from the inside while Martin drives off a road after nearly crashing into an Ankylosaurus and is devoured by the Distortus. Duncan distracts the beast with a flare as Zora retrieves the samples, allowing the group to escape by boat. In the end, Zora and Henry decide to distribute the biomaterial as open-source, ensuring global access.
