Salt water aspiration syndrome happens when a diver breathes in small amounts of salt water, which irritates and inflames the lungs. It’s different from drowning or near-drowning, where large volumes of water enter the body. The problem often stems from faulty gear—like a bad regulator—or poor breathing technique, which lets tiny water droplets slip past the upper airway’s defenses and reach the lower lungs.
The first sign is a cough, sometimes bringing up excess or blood-tinged mucus. About two hours later, symptoms like shortness of breath, rapid breathing, a fast heart rate, bluish skin, and chest pain or discomfort set in. It can feel like a viral infection, with fever, chills, and malaise often appearing first. Many people also get tremors and shivering, which get worse if blood oxygen is low or if they’re cold or active. Headaches happen in roughly two-thirds of cases, and body aches in about a third. Some experience nausea, vomiting, or loss of appetite. Less common are fainting, brief loss of consciousness, or mild confusion. Rarely, severe lung inflammation occurs. Doctors might hear abnormal breath sounds like snoring or rattling, and chest scans can show fluid buildup or lung tissue damage. Lung function tests often reveal a drop in volume—around 0.7 liters less in forced expiratory volume and vital capacity. Blood tests aren’t specific but may show low oxygen, slightly high white blood cells, and elevated lactate dehydrogenase.
Diagnosis relies on a person’s history and physical exam. The cough and breathing trouble can mimic near-drowning, but near-drowning doesn’t cause viral-like symptoms. It can also be confused with a viral infection at first, but salt water aspiration syndrome usually improves within hours. Decompression sickness is another possibility; checking the dive profile and ruling out other decompression symptoms helps tell them apart. A big improvement with supplemental oxygen strongly points to this syndrome. Other diving issues—like lung barotrauma, immersion pulmonary edema, or hypothermia—can look similar or happen at the same time, but the clinical course and imaging usually clarify things.
Treatment is supportive: oxygen, observation, and rest. Most people recover within 24 hours, often without any treatment. Severe cases, or those complicated by existing lung problems, may need critical care.
