Defense Secretary Dick Cheney tried to kill funding from 1989 to 1992, but Congress provided money anyway. Studies showed the V-22 offered more capability at similar costs. The Clinton administration supported it, helping secure funding. The Army later chose a different tiltrotor, the V-280 Valor, to replace its UH-60 Blackhawk in the 2020s.

The first of six prototypes flew in helicopter mode on March 19, 1989, and in fixed-wing mode on September 14, 1989. The third and fourth prototypes completed successful tests. The complexity of being the first military tiltrotor caused a long development period. Several accidents raised concerns. The Marine Corps started crew training for the MV-22B in 2000 and fielded it in 2007, replacing its CH-46 Sea Knights. The Air Force fielded its CV-22B in 2009. Since then, Ospreys have been used for transport and medical evacuation in Iraq, Afghanistan, Libya, and Kuwait. The Navy began using the CMV-22B for carrier deliveries in 2021.
