Joe Barbaro is Vito Scaletta's childhood friend and accomplice. In Vito's absence during World War II, Joe joined Don Alberto Clemente's crime family. After Vito returns to Empire Bay, Joe supplies him with counterfeit discharge papers and introduces him to contacts for work. Later, Joe and Vito become made men within the Falcone family, led by Don Carlo Falcone. Joe is present when Vito and Henry Tomasino carry out jobs for the Clemente family, and later participates in the heroin deal with the Triads. After Henry is murdered by the Triads, Joe and Vito retaliate but fail to retrieve the money, leaving them indebted to loan shark Bruno Levine.
