The Fourth Doctor regenerated into this incarnation after falling from the Pharos Project Radio Telescope, merging briefly with a future self called the Watcher. The process was difficult; he initially displayed personality traits of all four previous incarnations before stabilizing in Castrovalva. His early travels focused on returning Tegan Jovanka to Heathrow Airport, though the TARDIS repeatedly missed the mark. A defining tragedy occurred when Adric died attempting to stop a space freighter from crashing into prehistoric Earth during the events of Earthshock. Later, the Doctor faced execution by the Time Lords after being temporally bonded with Omega in Amsterdam. The era also saw the return of classic foes like the Silurians and Sea Devils. Ultimately, on Androzani Minor, the Doctor and companion Peri Brown were exposed to toxic spectrox gas. With only one dose of antidote available, the Fifth Doctor sacrificed his own life to save Peri, expressing doubt that regeneration was possible before transforming into the Sixth Doctor.

The TARDIS crew faced constant peril, from the Black Guardian's plot to have schoolboy Vislor Turlough kill the Doctor, to the shape-shifting robot Kamelion impersonating King John. In one instance, the Doctor helped Kamelion regain his free will, only to later destroy him when the Master regained control of the robot in Planet of Fire. The Doctor also met three previous incarnations summoned to Gallifrey's Death Zone by President Borusa. His final adventure took place on Androzani Minor, where he and Peri were caught in a conflict over the drug spectrox. As the raw form proved deadly, the Doctor realized only one dose of the cure existed. He chose to drink it himself, ensuring Peri's survival while accepting his own death. Before regenerating, he voiced uncertainty about his ability to continue, marking a somber end to an era defined by loss and reluctant heroism.

The Fifth Doctor is distinguished by his Edwardian cricketer costume: a cream frock coat, striped trousers, plimsoll shoes, and a Panama hat with a red band. He frequently carried a cricket ball and wore spectacles not for vision but to appear clever, though he once noted short-sightedness in one eye. A defining accessory was the celery stalk on his left lapel; he claimed it would turn purple in the presence of 'Praxis' range gases to which he was allergic. If this occurred, he intended to eat it as a restorative for teeth and health. This incarnation displayed an unusually acute sense of taste, inherited by later Doctors. He also possessed a vulnerable personality, often hesitating in decision-making. Despite his gentleness, he faced the death of Adric and the departure of Tegan and Turlough.
