Kohr came through Bayer Leverkusen's youth system. He made his Bundesliga debut on 21 April 2012 in a 1–0 loss to 1899 Hoffenheim. He was loaned to FC Augsburg on 13 January 2014 for two seasons, and the loan was made permanent on 18 June 2015. He rejoined Leverkusen on 1 July 2017 after the club used a buy-back clause. On 3 July 2019, he joined Eintracht Frankfurt on a five-year deal. On 18 January 2021, he joined Mainz 05 on loan, which was extended for the 2021–22 season, and on 12 May 2022 he moved to Mainz 05 permanently on a four-year contract.
