The game takes place in an alternative version of the final days of World War II, with the German army close to defeat. Adolf Hitler deploys a last resort plan to raise the fallen as zombies through occult rituals and turn them against Allied forces, causing Germany to become overrun with the undead. The trilogy includes the single-player campaigns of the first two Nazi Zombie Army titles alongside a new third chapter, combined into one Campaign mode, with a Horde mode similar to Killing Floor and the Left 4 Dead series also available. The game's cooperative modes support eight playable characters, four of which are new female characters. In August 2015, the main characters of both games in the Left 4 Dead series were added exclusively to the PC version's playable character roster.

Gameplay utilizes mechanics similar to Sniper Elite V2, encouraging players to tackle enemies from afar with a sniper rifle rather than head-on, though with a more linear level design and reduced focus on stealth in favor of action. The player uses era-appropriate weapons, explosives such as grenades, tripwire mines, and dynamite, and must account for wind direction, wind strength, and bullet drop when sniping. Bullets can ricochet off surfaces or targets. The "X-Ray Kill Cam" returns, tracking a successful shot in slow motion and showing an anatomically correct x-ray view of the damage. Some items trigger explosions when shot, and notable shots are briefly measured by distance and factors like head shots or moving targets.

The plot follows OSS operative Karl Fairburne, Red Army infantryman Boris Medvedev, Wehrmacht Captain Hermann Wolff, and German occultist Efram Schweiger as they team up to stop Hitler's zombie horde. After retrieving the Book of Souls and the three pieces of the Sagarmatha Relic, the group confronts Hitler, who has risen from the dead as an undead leader. They ultimately defeat an enormous zombie Hitler, toss the real Hitler into Hell, and destroy his headquarters.
