Mehdi Hassan was born into a family of Kalawant musicians in the village of Luna, then in British India. He began singing at age 4–5 and started formal Hindustani classical training at age 8 under his father Ustad Azeem Khan and uncle Ustad Ismail Khan. By age 18, he had mastered dhrupad, dadra, thumri, and khayal. After the 1947 partition, his family migrated to Pakistan, where he initially worked as a bicycle and tractor mechanic while maintaining his singing practice.

His breakthrough in Pakistani film came in 1964 with the ghazal "Gulon mein rang bharay, baad-e-naubahar chale" from the film Farangi, written by Faiz Ahmed Faiz and composed by Rasheed Attre. The song's popularity was such that Faiz stopped reciting the ghazal at mushairas, saying it now belonged to Hassan. Hassan sang for over 300 films during his career. In 2010, Saregama released "Sarhadein" featuring his first and last duet with Lata Mangeshkar, "Tera Milna."

Following a severe illness in the late 1980s, Hassan reduced his singing and eventually stepped down from playback singing. He suffered strokes in 2000 and 2005, the latter leaving him with speech impairments. He died of multiple organ failure on 13 June 2012 at Aga Khan University Hospital in Karachi.
