Venue

The final was the fourth European Cup or Champions League final held in Portugal and the first to take place outside Lisbon, which had hosted the 1967, 2014 and 2020 finals. It was also the first time the competition's final was staged in the same country in consecutive seasons. The Estádio do Dragão, a 50,000-seat stadium, had previously hosted matches at UEFA Euro 2004 and the 2019 UEFA Nations League Finals. This final was the first since 2004 to be held in a stadium with a capacity below 60,000.

The original host for 2021 was the Atatürk Olympic Stadium in Istanbul, selected by the UEFA Executive Committee in May 2018 after an open bidding process. However, due to the postponement and relocation of the 2020 final to Lisbon because of the COVID-19 pandemic, Istanbul was shifted to host the 2021 final. Two weeks before the match, UEFA relocated the final to Porto to allow a limited number of fans to attend. A capacity limit of 33 percent was agreed, resulting in an attendance of 14,110.

Premier League side Aston Villa had offered Villa Park in Birmingham as an alternative venue, citing travel limitations related to the pandemic in Turkey. Villa Park had previously hosted the 1999 UEFA Cup Winners' Cup Final and the 2012 FA Community Shield between Manchester City and Chelsea.

Pre-match

UEFA appointed Spanish referee Antonio Mateu Lahoz for the final on 12 May 2021. He had been a FIFA referee since 2011 and served as fourth official in the 2019 final. He officiated six matches in the 2020–21 Champions League season and had worked at the 2018 FIFA World Cup and was selected for UEFA Euro 2020. His fellow countrymen Pau Cebrián Devís and Roberto Díaz Pérez del Palomar were assistant referees, with Carlos del Cerro Grande as fourth official. Alejandro Hernández Hernández acted as video assistant referee, assisted by Juan Martínez Munuera, Íñigo Prieto López de Cerain and Polish referee Paweł Gil. American DJ and electronic music producer Marshmello performed a virtual opening ceremony show with Selena Gomez and Khalid.

Match

The match kicked off around 8 p.m. before a crowd of 14,110. In the 15th minute, Mason Mount set up Werner, whose shot was saved by Ederson. N'Golo Kanté headed over from a Ben Chilwell cross. Phil Foden was tackled by Antonio Rüdiger when about to shoot. İlkay Gündoğan received the first booking for a foul on Mount.

In the 38th minute, Kanté played to Havertz, who was denied by Oleksandr Zinchenko. Chelsea made the first substitution when injured Thiago Silva was replaced by Andreas Christensen. Three minutes before half-time, Mount passed to Havertz, who went one-on-one with Ederson; the goalkeeper deflected the ball, but Havertz scored into an empty net for a 1–0 lead. Three minutes of stoppage time were added.

No changes were made at half-time. Manchester City dominated early in the second half. Rüdiger was booked for a foul on Kevin De Bruyne, who left the pitch in tears under concussion protocol and was replaced by Gabriel Jesus. City appealed for a penalty when the ball struck Reece James on the arm, but VAR ruled no foul as the ball deflected off James' chest first. Bernardo Silva was replaced by Fernandinho, and Christian Pulisic came on for Werner.
