Background
In June 1998, North Jersey mobster Tony Soprano, a captain in the DiMeo crime family, is referred to the psychiatrist Dr. Jennifer Melfi after a panic attack. When a Czech-American organization bids against Tony for a waste management contract, Christopher Moltisanti murders one of the Czechs, Emil Kolar, and the Czechs withdraw. Tony's uncle Corrado "Junior" Soprano wants to kill "Little Pussy" Malanga in Artie Bucco's restaurant, so Tony has Silvio Dante bomb the restaurant instead.
A confrontation with his mother Livia, who resists moving into a retirement home, triggers another panic attack; guided by Melfi, Tony realizes he was sad to see the ducks leave his pool because he dreads losing his own family. Driving Livia to A.J.'s birthday party, Junior says that "something may have to be done about Tony", and Livia shows a hint of a smile.
