Pathologic is set in a small, remote town where a strange lethal sickness called the sand plague has broken out. The player selects one of three healers—the Bachelor, the Haruspex, or the Devotress—each with their own questline and a group of NPCs called their 'Bound' whom they must keep safe by completing main quests each day. The game's world is perpetual: quests are erased at midnight each day, and the story continues regardless of whether they are completed. The town's districts and major buildings are named after body parts and biology; on one edge stands the Polyhedron, a physically impossible structure used as a fortress by children, and on the opposite side lies an Abattoir with a dormitory called the Apiary (later re-translated as 'Termitary'). The economy fluctuates to represent supply and demand in an isolated, disease-ridden environment.
