Mafia: The Old Country is a 2025 action-adventure game developed by Hangar 13 and published by 2K. It is the fifth main entry in the Mafia franchise, and a prequel to the series, taking place decades before the events of the first game. Set in Sicily, Italy, in the early 1900s, the game follows Enzo Favara, a teenaged carusu who becomes embroiled in the rise and fall of the fictional Torrisi crime family. The game was released for PlayStation 5, Windows, and Xbox Series X/S on August 8, 2025.

Lore & Background

Mafia: The Old Country is set in Sicily during the 1900s in and around the fictional town of San Celeste, which was previously featured in Mafia II. The narrative follows Enzo Favara, who journeys from a childhood of indentured labor as a carusu in Sicily's sulfur mines to joining the crime family of Don Bernardo Torrisi. The plot spans 1904 to 1907, covering Enzo's rise in the Torrisi family, a war with the Spadaro family, and his eventual disillusionment with the criminal lifestyle. A DLC story expansion titled Man of Honor was released on August 14, 2026.

In Their Own Story

The road out of San Casciano is gravel and mud, and Enzo walks it with a leather satchel that holds three things: a letter of introduction he does not fully understand, a knife his mother pressed into his hand at the village gate, and a photograph of a man he has never met. Behind him, the church bell rings for the dead. Ahead, the coast is a grey smear in the fog, and somewhere beyond it, a ship is waiting to take him to a country where his name will mean something different. He does not know yet that the code he learned in the back rooms of the village tavern — the code of silence, of obligation, of the hand that feeds you and the hand that cuts you — will follow him across the ocean and into a city of steel and smoke. He only knows that staying means dying in a trench for a king he has never seen, and leaving means becoming someone he cannot yet name.

Reader's Guide

Mafia: The Old Country is a third-person linear, narrative-driven game set in cinematic environments. This brings the game more in alignment with Mafia and Mafia II rather than the more open-world oriented Mafia III. While primarily linear, the player can diverge from the primary path during certain story chapters to explore the environment, or sometimes to reach optional objectives.
