Premise

Based on the British show Little Angels, Nanny 911 sends British nannies to reform American families with out-of-control children. Each episode opens with clips of unruly kids and shocked nannies, followed by a call to Head Nanny Lilian, who answers an old rotary phone. After the intro, the featured family is shown in a separate clip. Head Nanny Lilian, along with Nanny Deb, Nanny Stella, and Nanny Yvonne, reviews the footage, and Lilian assigns the nanny best suited to the family's main issue.

The chosen nanny arrives in traditional dress. The first day is for observation only, to create a behavior plan; that evening, the nanny explains the needed changes. The rest of the week shows the plan in action, often with negative parental reactions. At the end, the nanny usually praises the family, and they receive a large gift.

This gift was dropped after season two. In season four, Nanny Yvonne was absent, and some episodes extended the observation period to two days.
