Illithid Tadpole

A parasitic organism that grants psionic powers but threatens your soul.

The Illithid Tadpole is a parasitic organism that infects the brains of humanoid hosts. In Baldur's Gate 3, the party seeks to cure themselves of the Illithid parasites infecting their brains. The tadpoles are implanted by Mind Flayers and, in the standard lifecycle, would trigger ceremorphosis, turning the victim into a new Mind Flayer. However, the tadpoles carried by the protagonist and their companions are stabilized by a mysterious divine force, halting the transformation temporarily and granting the hosts powerful psionic abilities.

Background

In the standard lifecycle of an Illithid, a tadpole is implanted into a host's brain. Over time, it consumes the mind and rewrites the biology, turning the victim into a new Mind Flayer. This process, known as ceremorphosis, is usually fatal to the original personality and physical form. However, the tadpoles found in Baldur's Gate 3 are anomalies. They were intended to be part of a grand ritual by the Absolute cultists to awaken the Netherbrain. The intervention of an unknown power prevented the full metamorphosis, leaving the hosts alive but connected to a psychic network. This connection allows them to hear the whispers of the hive mind and access latent psionic talents that would otherwise remain dormant.
