Production
Fox first announced it was developing an American version of the Italian medical drama Doc – Nelle tue mani in February 2023. The series, created by Francesco Arlanch and Viola Rispoli for Italy's Rai 1, was ordered straight to series that April. Barbie Kligman created the adaptation and executive produces alongside Hank Steinberg and Erwin Stoff. Production companies include 3 Arts Entertainment, Fox Entertainment, and Sony Pictures Television. The show was originally slated for the 2023-24 season but got pushed to 2024-25 because of delays caused by the 2023 Hollywood labor disputes. Fox renewed it for a second season of 22 episodes in February 2025, making it the network's first drama to get a full 22-episode order since The Resident in the 2021-22 season. A third season, also 22 episodes, was ordered in March 2026.
Molly Parker was cast as lead Dr. Amy Elias in December 2023. The following March, Omar Metwally, Amirah Vann, Jon-Michael Ecker, and Anya Banerjee joined as series regulars, with Scott Wolf and Patrick Walker set to recur. Walker and Charlotte Fountain-Jardim were promoted to regulars for season two in May 2025. Felicity Huffman became a new series regular that June, and Emma Pfitzer Price was cast in a recurring role that August. For season three, Blair Underwood was added as a regular in March 2026, while Barrett Carnahan, Raffa Virago, Cecilia Lee, and Kolton Stewart were cast in recurring roles that August.
Production kicked off in March 2024 in Toronto and Oakville, Ontario. Filming for season two ran from June 6, 2025 to April 3, 2026. Season three began filming on June 29, 2026 and is expected to wrap on April 2, 2027.
