In a land of false prophets, the Lamb, supposedly the last of their kind, is brought before the Four Bishops of the Old Faith and sacrificed. Upon dying, the Lamb is brought before 'The One Who Waits', a strange imprisoned deity who tasks the Lamb with starting a cult in its name, gives the Lamb a demonic crown, and resurrects it. Assisted by Ratau, the Lamb's precursor, the Lamb settles at the ruins of a temple and establishes a cult on behalf of The One Who Waits, in order to defeat the four Bishops—Leshy, Heket, Kallamar, and Shamura—and free the deity. The game is primarily split between crusades—action sequences working through dungeons—and calmer sequences of cult management. The player can manage followers in the cult's base, assigning tasks such as gathering resources, building structures, worshipping, sending them to assist in battle, or sacrificing them. Followers have appearances that can be changed and both positive and negative traits, which can affect how they act within the cult and how they respond to the player's actions. The game also features Twitch integration via the 'Companion of the Lamb' extension, allowing viewers to customize followers, contribute channel points for random rewards, and vote to help or hinder the player.
