Miller grew up in East Lansing, Michigan, and started youth hockey in California, originally playing as a forward. He switched to goaltending after becoming frustrated with his team's goaltender, and his father agreed to buy him a catching glove if he scored two goals and three assists in the next game—which he did. He played three seasons at Michigan State University before moving to the AHL with the Rochester Americans, where he tied Gerry Cheevers' record with 41 wins in the 2004–05 season. Miller established himself as the Sabres' starting goaltender in 2005–06, leading the team to the conference finals. He was voted the starting goaltender for the Eastern Conference in the 2007 NHL All-Star Game and gained a reputation as a shootout specialist, going 10–4 in shootouts that season. Miller broke the Sabres franchise record for most games played in a season (76) in 2007–08 and surpassed Dominik Hašek as the franchise leader in career wins (235) in 2012.

Miller's significance lies in his Vezina Trophy win in 2010, making him the first American player to also win the NHL Foundation Player Award that same night. He was the winningest American-born goaltender in NHL history from February 2019 until March 2024, when Jonathan Quick passed him. Miller represented the United States as starting goaltender in the 2010 Winter Olympics, winning a silver medal and being named tournament MVP, and also played in the 2014 Winter Olympics. He is one of 39 goaltenders in NHL history to reach 300 career wins. His hybrid style of goaltending and his success against the Toronto Maple Leafs earned him the nickname 'Leaf Killer: Ryan Miller' from Toronto media. Miller's #30 was retired by the Buffalo Sabres, and he was inducted into the Buffalo Sabres Hall of Fame in January 2023 and the Rochester Americans Hall of Fame in April 2025.
