Carnevale was born to Gaetano and Filomena Carnevale as one of seven children. He learned carpentry in Monte San Biagio to bring money home. He played football with his brothers Enzo and Germano on an improvised field. He was scouted by Latina Calcio 1932, where Alessandro Altobelli had played.

After playing for various Italian teams, he achieved success with Napoli from 1986 to 1990, playing alongside Diego Maradona and Careca, winning two Scudetti and one Coppa Italia. He scored four goals after transferring to Roma in 1990, but was suspended for one year due to illegal drug use alongside teammate Angelo Peruzzi. He spent two more years in Rome before playing three seasons in Serie B for Pescara (twice) and Udinese. Carnevale represented Italy ten times with two goals between 1989 and 1990.

He debuted on 22 April 1989 in a 1–1 friendly draw against Uruguay in Verona. He scored his first international goal on 26 April 1989 in a 4–0 victory against Hungary in Taranto, and his second on 20 September 1989 in a 4–0 friendly victory against Bulgaria in Cesena. He was included in Italy's squad at the 1990 FIFA World Cup, where they finished third. He was a regular starter in pre-tournament friendlies and appeared in the first two group stage matches.

He was replaced by Salvatore Schillaci in Italy's opening game against Austria, and Schillaci scored two minutes later. Carnevale started the next game against the United States but was replaced by Schillaci after failing to score. After the tournament, he was no longer called up. He also played for Italy at the 1988 Summer Olympics, where they finished fourth.

Carnevale played as a forward. His club honours with Napoli include Serie A titles in 1986–87 and 1989–90, and the Coppa Italia in 1986–87. He also won a UEFA Cup with Napoli in 1988–89. Internationally, he earned a FIFA World Cup third-place finish in 1990. He was awarded the 5th Class / Knight: Cavaliere Ordine al Merito della Repubblica Italiana in 1991.
