Ketamine

Ketamine is a cyclohexanone-derived general anesthetic and NMDA receptor antagonist with analgesic and hallucinogenic properties. It is used medically for anesthesia, depression, and pain management, and is on the World Health Organization's List of Essential Medicines. First synthesized in 1962, ketamine was developed from phencyclidine in an effort to create a safer anesthetic with fewer hallucinogenic effects. It received approval for medical use in the United States in 1970 and was extensively employed for surgical anesthesia during the Vietnam War, as well as in veterinary medicine. A major breakthrough occurred in 2000 with the discovery of its rapid antidepressant effects, which marked a significant advance in treating depression. Psychotomimetic side effects can be reduced by co-administration of lamotrigine, nimodipine, benzodiazepines, or propofol; a combination of ketamine and propofol is known as ketofol.

At lower, sub-anesthetic doses, ketamine is used for pain management and treatment-resistant depression. In emergency departments and perioperative settings, ketamine infusions reduce morphine use, pain levels, nausea, and vomiting after surgery, and are especially beneficial for opioid-tolerant patients or those expected to have severe post-operative pain.
