Dempsey was born in Nacogdoches, Texas, and is of Irish ancestry on his father's side. For much of his childhood, his family lived in a trailer park where he grew up playing soccer with Hispanic immigrants. His older brother Ryan brought him to a tryout for the Dallas Texans, where Dempsey was noticed while juggling a ball on the sidelines. He had to quit due to financial constraints but rejoined after other parents offered to help with expenses. On November 27, 1995, his 16-year-old sister Jennifer died from a brain aneurysm, which deepened his motivation to pursue soccer in her honor. He studied the play of Argentina's national team, especially Diego Maradona, and was heartbroken when Maradona did not play in the 1994 World Cup game in the Cotton Bowl. He attended Furman University as a health and exercise science major and was a key player for the Paladins soccer team.

Dempsey was drafted by the New England Revolution with the eighth overall pick in the 2004 MLS SuperDraft. He made his debut on April 17, 2004, and scored his first MLS goal the following week. In his rookie season, he started 23 of 24 matches, scored seven goals, and earned MLS Rookie of the Year honors. He was named to the MLS Best XI in 2005 and 2006. In 2007, he signed for Premier League club Fulham for £2 million, becoming the most expensive American export to the Premier League at the time. He scored his first goal for Fulham on May 5, 2007, a 1-0 win over Liverpool that saved Fulham from relegation. He was awarded Fulham Player of the Season for 2010–11 and 2011–12, and is Fulham's all-time top Premier League goalscorer with 50 goals. In 2012, he became the first American to score a hat-trick in the Premier League. He then signed for Tottenham Hotspur for $9.6 million, scoring 12 goals across all competitions in one season. He was signed by the Seattle Sounders in 2013, playing 115 matches and scoring 47 goals, leading the club to win the 2014 Supporters' Shield. During the 2016 season, he was diagnosed with an irregular heartbeat and missed the team's run to the MLS Cup. He returned the following season, winning the 2017 MLS Comeback Player of the Year Award before retiring in 2018.
