Hölzenbein debuted for Eintracht Frankfurt in the Bundesliga in 1967 and served the club until his departure in 1981. He won the DFB-Pokal with Frankfurt in 1973–74, 1974–75, and 1980–81, and the UEFA Cup in 1979–80. He later played for Fort Lauderdale Strikers, Memphis Americans, and Baltimore Blast. At international level, he was a member of the West German team that won the 1974 World Cup, making six appearances in the tournament including the final, where he was fouled leading to the equalizing penalty. He also scored the equalizing goal in the UEFA Euro 1976 Final, which West Germany lost on penalties.
