Gostisbehere was born in Pembroke Pines, Florida, and began playing hockey with the youth affiliate of the Florida Panthers. He transferred from Marjory Stoneman Douglas High School to South Kent School in Connecticut, then played three seasons of college hockey at Union College, helping the team win its first NCAA championship in 2014. He was named the tournament's Most Outstanding Player and was a finalist for the Hobey Baker Award. He also helped the United States national junior team win a gold medal at the 2013 World Junior Championships.

Selected by the Flyers in the third round of the 2012 draft, Gostisbehere signed an entry-level contract in 2014. After a brief stint with the AHL's Adirondack Phantoms, he suffered an ACL tear that sidelined him for most of the 2014–15 season. The following fall, he began a record-breaking season with a 15-game point streak, the longest by any rookie defenseman in NHL history. He became the youngest Flyer to receive the Barry Ashbee Trophy for best team defenseman and was a runner-up for the Calder Memorial Trophy. He represented Team North America at the 2016 World Cup of Hockey.

A recurring knee injury began to affect his performance after October 2019, leading to multiple arthroscopic surgeries. After being traded to Arizona in July 2021 due to salary cap constraints, he later signed a three-year, $9.6 million contract with the Carolina Hurricanes on July 1, 2024, and won the Stanley Cup with them in 2026.
