His permanent death occurs much later in the story, during the Echidna arc (around Arc 18, 'Queen'). Skitter kills him by filling his lungs with bugs after he teleports into her swarm during the attack on the PRT headquarters. He was never a member of the Slaughterhouse Nine, nor was he hired to assassinate Danny Hebert. His power creates a duplicate that collapses into ash while he teleports to a previous location as part of his normal ability, not specifically upon death.
