Puspa was born as Sudarwati in Tanjung, Dutch Borneo, on 1 November 1937, to Tugeno Puspowidjojo and Siti Mariam. Her family later changed her name to Kadarwati and then Sumarti, following cultural customs to aid her recovery from illness. She was the fourth of twelve children. After winning several singing competitions, she decided around age 14 to become an entertainer, despite her parents' forbidding it. She considered her career to have started in 1954 when she won a Radio Republik Indonesia singing competition in Semarang. Her stage name was given by her father.

In 1959 she moved to Jakarta, made her first recording for Lokananta, and began working as a lounge singer at Hotel Des Indes. She sang with the Jakarta Symphony Orchestra from 1960 to 1962. Puspa won the National Pop Song Festival in 1974 with her song 'Cinta' and represented Indonesia at the World Popular Song Festival in Tokyo that year, reaching the semi-finals. In 1984 she won the Bronze Prize at the World Popular Song Festival in Tokyo with 'Horas Kasih'. She was a palace singer for Sukarno and Suharto and participated in cultural missions to Malaya (1960) and the Netherlands (1965).

Puspa also acted in films from the mid-1960s into the 1980s, including 'Minah Gadis Dusun' (1965), 'Dibalik Tjahaja Gemerlapan' (1966), 'Bawang Putih' (1974), 'Tiga Cewek Bandung' (1975), and 'Apanya Dong' (1969). She composed operettas in the 1970s and founded the arts organization Papiko in 1972. She was married three times: to Kasno (1957–1959), Zainal Ardi (1960–1963), and Mus Mualim (until his death in 1991). She learned songwriting from Mualim. In 2009 she was diagnosed with cervical cancer but was declared cancer-free after treatment.

Titiek Puspa is regarded as a major figure in Indonesian popular music, often called the grand dame of Indonesian entertainment. Her career spanned from the 1950s into the 2020s, and she was noted for her distinctive alto voice and her songwriting that addressed social issues such as poverty, prostitution, and women's lives. Her song 'Kupu-Kupu Malam' (1977) sympathetically portrayed a sex worker, challenging societal stigma. Four of her songs were listed by Rolling Stone Indonesia in 2009 among the best Indonesian songs of all time, including 'Kupu-Kupu Malam' (ranked 32nd), 'Bing' (41st), 'Gang Kelinci' (66th), and 'Mari-Mari' (113th). She received a Special Award from ASIRI in 2017 for her lifetime dedication to the music industry. Her work as a palace singer for two presidents and her participation in international song festivals underscored her national and international reach. A biography by Alberthiene Endah, titled 'A Legendary Diva', was released in 2008.
