Pechstein began her Olympic career in 1992, winning bronze in the 5000 metres. She went on to win gold in that event in three consecutive Olympics (1994, 1998, 2002), and added a fifth gold in the team pursuit in 2006. She also won medals in the 3000 metres: silver in 1994, gold in 1998, and bronze in 2002. After missing the 2010 Vancouver Games due to a doping ban, she returned for the 2014 Sochi Games, finishing fourth in the 3000 metres and fifth in the 5000 metres. In February 2009, following the World Championships in Norway, the International Skating Union accused Pechstein of blood doping based on irregular reticulocyte levels and banned her for two years. Pechstein denied doping, arguing that her reticulocyte percentage of 3.49 fell within normal range for women her age and that the ISU's threshold of 2.4 was too low. She also cited an inherited condition, spherocytosis, which was later suggested by a German hematology expert. The Court of Arbitration for Sport upheld the ban in November 2009, and the Swiss Federal Supreme Court rejected her appeal in September 2010. No forbidden substances were ever found. Pechstein returned to competition in February 2011 and won bronze in the 5000 metres at the 2011 World Championships.

Pechstein pursued legal action for damages against the ISU. In June 2016, the lower Federal Court of Justice of Germany rejected her initial appeal, but on 3 June 2022, the higher Federal Constitutional Court of Germany ruled that her fundamental rights had been violated, sending the case back for re-trial. She also expressed interest in competing as a cyclist, planning to race in the individual pursuit at the German Track Championships in July.
