Pietrangelo played minor hockey in the Greater Toronto Area, including with the Vaughan Kings of the GTHL. He was drafted third overall by the OHL's Mississauga IceDogs in 2006 and later played for the Barrie Colts. He was selected fourth overall by the St. Louis Blues in the 2008 NHL Entry Draft. He signed a seven-year, $45 million contract with the Blues in 2013 and was named captain of the Blues on August 24, 2016. On June 12, 2019, the Blues defeated the Boston Bruins to win the Stanley Cup; Ryan O'Reilly scored the Cup-winning goal in game seven. On October 12, 2020, he signed a seven-year, $61.6 million contract with the Vegas Golden Knights. He won his second Stanley Cup with the Golden Knights in 2023.

Pietrangelo is a defenceman and alternate captain for the Vegas Golden Knights. He has been named to the NHL Second All-Star Team twice (2011–12 and 2013–14) and selected to three NHL All-Star Games. He won the All-Star passing challenge in 2018 and the Breakaway Challenge in 2022. He led all defencemen in scoring during the 2019 playoffs and set a Blues franchise record for most assists by a defenceman (326) on December 29, 2019.
