The final fight of the original game's eight floors: the heart of Isaac's mother.
Mom's Heart is the boss at the end of the eighth floor of The Binding of Isaac. After the player defeats Isaac's mother on the sixth of the game's eight floors, Isaac crawls into her womb; the later floors are significantly harder and culminate in a fight against her heart.

Lore & Background
The original game has no single conclusion: it offers 13 endings, one after each major boss fight, with the first ten introducing unlocked items and mechanics and the final three hinting at the story's meaning. Rebirth's endings suggest that Isaac, from overwhelming religious guilt, locked himself in a toy chest and asphyxiated, with the game's events a hallucination.
Reader's Guide
The heart is fought on the eighth floor. Beyond it lies an optional ninth floor, Sheol, with the boss Satan. In Rebirth a run is completed by beating one of a number of different final bosses, and seeded runs do not count toward achievements.
