Synopsis

In 2017, an extraction of 47 Korean criminals from the Philippines turns chaotic when an elderly man detonates a bomb, killing seven officers and wounding about 30 travelers. Five years later, a similar group of criminals is transported via cargo ship to Korea to prevent a repeat. After the detectives walk the prisoners onto the ship, the Busan VTS is ordered to track the vessel. Onboard, law enforcement led by Lee Seok-woo confronts the criminals. A confrontation with Lee Do-il (Tattoo) escalates into violence. Meanwhile, Lee Seok-woo (Doctor) loads a bag of syringes and medical equipment under the guise of food, goes below deck, and administers a sedative to a prisoner in a jumpsuit with a breathing apparatus. Chaos ensues as Tattoo's gang picks their handcuffs and launches a violent spree. A series of brutal killings reveals Alpha, a superhuman with extraordinary strength and resilience, whose origin links to disturbing World War II experiments. Alpha goes on a rampage, confronting Park Jong-doo and his team, resulting in Cop Leader's demise. The film reveals Alpha's transformation due to inhumane experiments. In the climax, Kyung-ho (Commander) arrives, revealing superhuman strength and a history with Lee. A battle ensues, with a revelation that Kyung-ho's past actions cost the life of Lee's wife and child; Lee was a gangster saved and experimented on by Kyung-ho, with few survivors. The doctor attempts to escape but is killed by one of Kyung-ho's team members.
