Fernandez was born in Maasin, Leyte as the youngest of nine siblings. He began playing basketball at age five, influenced by his older brothers. After studying at a seminary for two years, he graduated from University of San Carlos in Cebu, playing for their Warriors from 1970 to 1972. He joined the San Miguel Braves in 1972, then transferred to the Komatsu Komets (later Toyota Comets) in 1973. When Toyota joined the PBA in 1975, Fernandez joined them, foregoing plans for NCAA or UAAP stints.

Fernandez played for five PBA teams: Toyota, Manila Beer (Beer Hausen), Tanduay, Purefoods, and San Miguel. He won nine titles with Toyota from 1975–1983, three with Tanduay from 1986–1987, and was a key part of San Miguel's grand slam in 1989. He won MVP awards with four different teams: Toyota (1982), Beer Hausen (1984), Tanduay (1986), and San Miguel (1988). He retired in 1994 after 20 seasons, holding multiple all-time PBA records.

After retiring, Fernandez served as commissioner of the Metropolitan Basketball Association (1998), the Collegiate Champions League (2003), and the United Regional Basketball League (2004). In 2016, he was appointed as one of four commissioners of the Philippine Sports Commission. He also ran unsuccessfully for a Senate seat in 1995 under the Nationalist People's Coalition.
