Demaría was born in Buenos Aires. He played 295 games for Internazionale of Italy between 1931 and 1943, scoring 86 goals for the club, also serving as the team's captain between 1940 and 1943. He also played for Gimnasia de La Plata, Estudiantil Porteño and Club Atlético Independiente in Argentina and Novara, Legnano and Cosenza in Italy. His younger brother Félix Demaría also played football professionally, including a season for Ambrosiana-Inter. To distinguish them, Attilio was referred to as Demaría I and Félix as Demaría II. Attilio Demaría died in Haedo, Buenos Aires on 11 November 1990.

The San Siro crowd rose as Attilio Demaría took the armband in 1940, becoming captain of Ambrosiana-Inter. He had already scored 86 goals in 295 appearances for the club, a tally built on clever movement and precise finishing. During the 1939–40 season, Demaría helped lead Inter to the Serie A title, adding to the Coppa Italia they had won the previous year. These were the club's first major trophies since the early 1930s. On the international stage, Demaría had already made history. He represented Argentina at the 1930 World Cup, earning a runners-up medal. Then, after switching allegiance to Italy, he was part of the squad that won the 1934 World Cup on home soil. He also contributed to Italy's victory in the 1933–35 Central European International Cup. In total, he made thirteen appearances for Italy between 1932 and 1940, scoring three goals. His career was a bridge between two footballing nations, and his leadership at Inter during a turbulent era cemented his place in the club's history.
