Clonorchis sinensis, the Chinese liver fluke, is a parasitic flatworm belonging to the class Trematoda, phylum Platyhelminthes. It infects fish-eating mammals, including humans, where it inhabits the common bile duct and gall bladder, feeding on bile. Endemic to Asia and Russia, it is the most prevalent human fluke in Asia and the third-most in the world, with about 85% of infections occurring in China. The infection, called clonorchiasis, can cause jaundice, biliary inflammation, bile duct obstruction, liver cirrhosis, and bile duct cancer, leading the International Agency for Research on Cancer to classify it as a group 1 biological carcinogen in 2012.

The parasite was discovered in 1875 by British physician James McConnell at the Medical College Hospital in Calcutta. He recovered the fluke from a 20-year-old Chinese carpenter who died on 8 September 1874, observing a swollen liver and distended bile ducts blocked by 'small, dark, vermicular-looking bodies.' McConnell published his observations in The Lancet on 21 August 1875. The formal scientific description was published in 1875 by Thomas Spencer Cobbold, who named it Distoma sinense. Subsequent reclassifications included Rudolf Leuckart naming it Distomum spithulatum in 1876, and Arthur Looss creating the genus Clonorchis in 1907, based on the highly branched testes.

Clonorchis sinensis is a major causative agent of bile duct cancer and remains actively transmitted in China, Japan, Korea, Russia, and Vietnam. Its lifecycle involves three hosts: freshwater snails as first intermediate hosts, freshwater fish as second intermediate hosts, and mammals as definitive hosts. The eggs are released through the biliary tract and excreted in feces; when deposited in water, they are eaten by snails. Inside the snail, the miracidium hatches and undergoes metamorphosis into a sporocyst, then rediae, and finally cercariae, which bore out and infect fish by penetrating their muscle, forming metacercariae. Humans become infected by eating raw or undercooked fish containing metacercariae. The fluke's ability to cause chronic liver disease and cancer underscores its public health significance in endemic regions.
