It is a prequel set 2,000 years before the events of Sacred, taking place in a fantasy setting. The plot revolves around T-Energy, a volatile substance that is the source of all life and magic in Ancaria. Originally controlled by the Seraphim, they later gave some control to the High Elves, who became the dominant race. A power struggle between the nobility and clergy of the High Elves over T-Energy allows other races to vie for control, causing T-Energy to go out of control and mutate creatures, destroy cities, and render regions uninhabitable. The player chooses a Light or Shadow campaign, determining whether they heal the land or intensify the chaos.

Sacred 2: Fallen Angel is significant as a prequel that expanded the Sacred universe with a large open world of 22 square miles, minimal loading times, and a day/night cycle affecting NPC behavior. It introduced seven fixed-gender classes, including the returning Seraphim and new classes like the Shadow Warrior and Temple Guardian. The game offered two campaign paths (Light and Shadow) that intersect, and a multiplayer mode with hot-swapping between single-player and multiplayer. Despite mixed reviews (71% on GameRankings for PC), it sold over 750,000 units worldwide by August 2009 and received a Gold certification in Germany.
