Rafalski grew up in Dearborn, Michigan, and played youth hockey in Detroit. After going undrafted, he played four seasons in Europe, winning the Pekka Rautakallio trophy as best Liiga defenseman in 1997 and 1999, the Finnish SM-liiga championship with HIFK in 1998, and the Kultainen kypärä in 1999 as the first non-Finnish player to win that award. He entered the NHL in 1999 as a free agent with the New Jersey Devils, where he was partnered on defense with captain Scott Stevens for five years. Rafalski helped the Devils win the Stanley Cup in 2000 and 2003, and was named to the NHL All-Rookie Team in 2000.

In 2007, Rafalski signed a five-year, $30 million contract with the Detroit Red Wings as an unrestricted free agent. He was teamed with Nicklas Lidström and helped the Red Wings win the Stanley Cup in 2008, scoring the first goal in game six of the Final. He scored the game-winning goal in the 2009 Winter Classic. Rafalski retired in 2011 due to knee and back injuries, walking away from the final year of his contract. He briefly returned to professional hockey in 2014 with the Florida Everblades of the ECHL but was released after 18 days due to back issues.
