Mills was born in Neptune City, New Jersey, and grew up in Long Branch, New Jersey. He attended Long Branch High School, where he was a standout football player and wrestler, winning District Championships in 1976 and 1977. Despite his athleticism, his 5'9" frame did not interest college scouts. He played college football at Montclair State College as a walk-on, becoming the all-time leader in career tackles (501), tackles in a season (142), and tackles in a game (22). He was a three-time NJAC First Team All-Star and named New Jersey Collegiate Writers Defensive Player of the Year in 1978. After going undrafted, he signed with the Cleveland Browns in 1981 but was released after preseason. He then signed with the Toronto Argonauts of the CFL in 1982 but was released before the season. He later joined the Philadelphia/Baltimore Stars of the USFL, where he became known as the 'Field Mouse' and anchored the 'Doghouse Defense,' leading the Stars to two USFL championships and earning three All-USFL selections. He followed coach Jim Mora to the New Orleans Saints in 1986, becoming a key member of the 'Dome Patrol' and earning four Pro Bowl appearances (1987, 1988, 1991, 1992). He signed with the Carolina Panthers in 1995, starting every game in the team's first three seasons, and earned a fifth Pro Bowl appearance in 1996 at age 37. He retired after the 1997 season. After playing, he became a defensive coaching assistant for the Panthers in 1998 and was promoted to linebackers coach in 1999. He was diagnosed with intestinal cancer in August 2003 and continued to coach, inspiring the team's run to Super Bowl XXXVIII with his 'keep pounding' speech. He died from cancer complications on April 18, 2005.

The locker room was quiet before the Panthers' victory over the Dallas Cowboys during their Super Bowl XXXVIII run. Sam Mills, diagnosed with intestinal cancer in August 2003 and told he had only a few months to live, stood before his teammates. He had continued to coach through chemotherapy and radiation. His plea to "keep pounding" became an emotional rallying cry that inspired the team. That speech later became the name of a fund to sponsor cancer research programs and an official team slogan. Mills continued to coach the team until his death from cancer complications on April 18, 2005.
