Difficulty Scaling
In the Mojave Wasteland, difficulty is not a setting—it is the empty canteen and the weight of every bullet in your pack.
Difficulty Scaling in Fallout: New Vegas refers to the optional Hardcore Mode, which adds survival mechanics the player must keep track of. Rather than a single toggle, the mode layers resource pressures on top of the existing combat and dialogue systems. The player must routinely eat, drink, and sleep to avoid dying from starvation, dehydration, or sleep deprivation. Healing items gradually heal wounds but crippled limbs can only be healed by doctors, any owned bed, or special medication; in Hardcore Mode, a Doctor's Bag or Hydra also works. Ammunition has weight, which necessitates careful inventory management, and companions can permanently die. Hardcore Mode must be selected at the start of a new game and cannot be toggled off once enabled; there is no achievement for completing the game with it enabled.

Lore & Background
The courier who walks the Mojave Wasteland in 2281 carries more than a Pip-Boy 3000. They carry the memory of a bullet to the head, the debt to Doc Mitchell who nursed them back to health, and the knowledge that the desert does not care about their level. Hardcore Mode was designed to make every canteen of water and every box of ammunition feel consequential. The game does not hold your hand. A courier who wastes three rounds on a radroach when those rounds are needed for the raiders ahead is making a choice the game will not forgive. Healing items gradually heal wounds, but crippled limbs require a doctor, any owned bed, or special medication; in Hardcore Mode, a Doctor's Bag or Hydra also works. Companions can permanently die. The difficulty in Fallout: New Vegas is the story told through consequence.
Reader's Guide
At its core, Fallout: New Vegas difficulty scaling is built on the optional Hardcore Mode. Healing items gradually heal wounds but crippled limbs can only be healed by doctors, a player-owned bed, or special medication. Hardcore Mode can be enabled or disabled at any point, but if the player completes the entire game with it enabled, they unlock an achievement. A concrete example: you approach a raider camp with four rounds in your gun. A direct assault might cost two or three shots per enemy. If you misjudge the count, you enter the fight with an empty magazine and must rely on melee or flee. In Hardcore Mode, the same four-round scenario is even less forgiving because every bullet weighs something. Systems that interact with difficulty include: the player's SPECIAL stats and skill points (a well-built courier can conserve ammo with headshots or avoid combat entirely through speech checks), the Pip-Boy's inventory management, and the reputation system (helping a faction might give you new armor or access to a secret base; harming one might send assassins).
