The game pits two factions, the Federation and the Empire, against each other. After Imperial forces discover plans for the unfinished Albatross, Generalissimo Killt decides to complete it. The Federation sends hero Super Joe to infiltrate, but he is captured. Spencer is then deployed to rescue Joe and finish the job. Spencer fights through enemy territory, defeats Imperial leaders, and eventually frees Joe from the bionic soldier Gottfried Groeder. Together, they destroy the Albatross and escape by helicopter as it explodes.
Gameplay centers on using Spencer’s bionic arm to grapple, swing, and climb. Players must chain grappling moves to cross hazards like wide gaps or electrified floors. Weapons and items are earned by defeating bosses, and some items (like flares) are needed to access certain areas. The game uses an overworld tactical map where players move a helicopter between areas and choose to infiltrate on foot. Enemy convoys can intercept the helicopter, forcing a top-down fight to destroy an anti-air truck. Compared to the original, the remake adds a health bar instead of a limited health pool, and health items restore health rather than increasing maximum hit points. Defeating enemies at full health earns bonus points. Spencer can now grab and throw oil barrels or use them as shields. Weapons can be switched instantly, and boss battles have been redesigned to require creative use of the arm and weapons—for example, latching onto a screw on a boss’s vehicle to expose a weak point. The final boss is a full level instead of a short sequence.

A sequel, Bionic Commando Rearmed 2, was released in February 2011.
