Barbashev began skating at age three and played for HC MVD in the Russian Junior Hockey League (MHL) during the 2011–12 season. He moved to North America in 2012, joining the Moncton Wildcats of the Quebec Major Junior Hockey League (QMJHL), where he scored 62 points in his first season and was named to the QMJHL All-Rookie Team. After being selected by the Blues in the 2014 draft, he signed a three-year entry-level contract and made his professional debut in the 2015–16 season with the Chicago Wolves of the AHL.

Barbashev made his NHL debut in 2017 and played his first full season with the Blues in 2018–19, recording 26 points in 80 games. He won his first Stanley Cup that year, contributing six points in 25 postseason games. After a difficult 2022–23 season with the Blues, he was traded to the Vegas Golden Knights on 26 February 2023 in exchange for prospect Zach Dean. He immediately found success in Vegas, playing on the top line with Jonathan Marchessault and Jack Eichel, and helped the team win the Stanley Cup with 7 goals and 11 assists in the playoffs.

Internationally, Barbashev has played for the Russian national junior team, winning a silver and bronze medal at consecutive World Junior Championships. He also won gold at the 2012 World U-17 Hockey Challenge and bronze at the 2014 World Junior Ice Hockey Championships.
