The OTs-38 Stechkin is a development in silent revolver technology, following the earlier Quiet Special Purpose Revolver (QSPR) used by US forces in Vietnam. Unlike the QSPR, which was a modified Smith & Wesson Model 29, the OTs-38 is purpose-built. Its SP-4 cartridge uses a captive piston to contain propellant gases, eliminating muzzle flash and report; the sound of live firing is claimed to be practically identical to dry firing.

The revolver's design is unusual: the barrel is mounted below the cylinder axis, so the gun fires from the lowest chamber, directing recoil straight into the shooter's hand rather than torquing the muzzle upward. The cylinder pivots out to the right. An integral laser sight is mounted above the cylinder axis, powered by three D-0.03D batteries. A manual safety catch allows the hammer to be carried cocked, and the mechanism prevents accidental discharge if the cylinder is open or not fully closed, or if the revolver is dropped or struck.
