The game follows the events of the first season of Fox's convict drama, but players control Tom Paxton, an agent of the covert organization The Company, led by Jack Mannix. Paxton goes undercover as a prisoner in Fox River State Penitentiary to ensure the execution of the falsely incarcerated Lincoln Burrows. The game is split into nine chapters, each representing a part of the TV series.

Prison Break: The Conspiracy was originally in development for PlayStation 3 and Xbox 360 for a February 2009 release, but was canceled when Brash Entertainment closed. Developer ZootFly continued the work, self-funding the project for 13 months. Once nearly finished, the game was acquired by new publisher Deep Silver. The game received generally unfavorable reviews on all platforms according to Metacritic, with The Daily Telegraph describing the Xbox 360 version as 'an abject failure on all counts.' Its significance lies in offering a different perspective on the television series' story, though it failed to achieve critical success.
