Toxoplasmosis is an infection caused by the parasite *Toxoplasma gondii*, which belongs to the apicomplexan group. The illness is linked to several neuropsychiatric and behavioral issues. Many people experience no symptoms, but some may develop mild, flu-like signs—such as sore muscles and swollen lymph nodes—that last a few weeks or months. In rare cases, eye problems can occur. For those with weakened immune systems, more serious symptoms like seizures and poor coordination may appear. If a pregnant woman becomes infected, the child can develop congenital toxoplasmosis.
Transmission typically happens through eating undercooked food that contains parasite cysts, contact with infected cat feces, or from a pregnant woman to her fetus. Spread via blood transfusion or organ transplant is uncommon. It is generally thought that the disease does not pass directly between people, though this is still debated. Studies have found *T. gondii* in human semen, suggesting sexual transmission might be possible, but this is not considered a main route. The parasite reproduces sexually only in cats, but it can infect nearly any warm-blooded animal, including humans. Diagnosis usually involves testing blood for antibodies or, in pregnant women, checking amniotic fluid for the parasite’s DNA.
Prevention focuses on proper food preparation and cooking. Pregnant women are advised to avoid cleaning cat litter boxes; if they must, they should wear gloves and wash their hands afterward. Healthy people generally do not need treatment. During pregnancy, medications such as spiramycin or a combination of pyrimethamine, sulfadiazine, and folinic acid may be used.
Up to half of the world’s population carries *T. gondii* without symptoms. In the United States, about 11% of people are infected, while in some regions the rate exceeds 60%. Around 200,000 cases of congenital toxoplasmosis occur each year. The organism was first described in 1908 by Charles Nicolle and Louis Manceaux. Transmission from a pregnant woman to her baby was first definitively documented in 1939 by Wolf, Cowen, and Paige. There is some evidence that even asymptomatic infection may alter behavior.
