The film follows Ichika Usami and her friends after she dreams of a girl named Sakura fleeing with a fox named Shizuku. Sakura arrives in Ichigozaka and seeks the Pretty Cure teams using three cards bearing symbols. The Kirakira Pretty Cure a la Mode team, joined by the Witchy Pretty Cure! and Go! Princess PreCure teams, help Sakura battle Akainu and Kiinu, two Shisa dogs serving the evil Karasu Tengu. A brainwashed Shizuku, transformed into Samidare, initially fights the Cures but is restored by Sakura's memories.

Pretty Cure Dream Stars! marks a shift in the crossover film series by focusing only on the three most recent Pretty Cure teams, a decision producer Takashi Washio made to avoid relying on previous generations. The film's theme of Japanese mythology and wa (harmony) was chosen to reveal the beauty and depth of Japanese culture, though Washio expressed concern it might distance children.
