Gameplay
Players choose between Naoki Kōsaka, a boy, or Rina Makimura, a girl, both trapped in a bus inside a tunnel after a large earthquake. Naoki is physically strong and self-confident but becomes stressed more easily, while Rina is physically weak but calmer, causing her stress to rise more slowly. The stress meter replaces the thirst and body temperature meters from earlier games, rising after angry conversations or witnessing traumatic incidents. Players choose responses during conversations, affecting the character's personality and stress levels. High stress impairs movement. Health drops from hazards like fire or gas leaks. Food and medicine replenish or reduce status bars; resting on a bench fully restores them and saves the game. Various outfits and compasses can be collected. Unlike the first two single-player games, 2-4 players can play cooperatively via ad hoc connection.
Plot
Naoki or Rina leaves home to start a new life as a university student in a Megalopolis, but an earthquake destroys the city and traps them in a tunnel. They must survive using wit, physical prowess, and luck, rescuing other survivors and seeking help. The player interacts with two female companions, Saki Honjō, a nurse turned amateur musician, and Ayami Hazuki, a high school girl. Their relationships affect the ending. Hazuki is held at gunpoint on a skyscraper by her half brother Yoichi, who seeks revenge for his daughter's murder. The player attacks as Saki distracts him; waves hit the building, causing Hazuki to lose balance. The player can save her or let her die. Letting her die yields a neutral bad end. Saving her with negative or neutral relationships leads to a bad end where both die. Positive relationships with either companion save both, leading to a good end where the player supports Saki's singing career or stays with Hazuki. Bad endings also result from abandoning companions or sacrificing oneself during cutscene choices.
