Lore & Background

Pathologic takes place in a remote town beset by a lethal sickness called the sand plague. The player chooses one of three healers—the Bachelor (Daniil Dankovsky), the Haruspex (Artemy Burakh), or the Devotress (Klara, later re-translated as the Changeling)—each with a unique questline and a group of Bound NPCs to protect. The game runs over twelve in-game days, with quests resetting at midnight; completing main quests is essential to uncovering the town's secrets and keeping key characters alive. The town's districts are named after body parts, and landmarks include the physically impossible Polyhedron and the Abattoir with its Apiary (later Termitary).

Pathologic was critically acclaimed in Russia, winning year-end accolades from five Russian outlets, but received mixed reviews in English-speaking countries due to poor translation, outdated graphics, and slow-paced gameplay. Its significance lies in its uncompromising design: a perpetual world where time passes regardless of player actions, a fluctuating economy reflecting supply and demand, and four endings tied to the survival of the Bound. The game was remastered as Pathologic Classic HD in 2015, and a remake of the Haruspex route was released as Pathologic 2 in 2019. A second remake reimagining the Bachelor route, Pathologic 3, was released on 9 January 2026. The game's legacy includes its influence on narrative-driven survival games and its cult following.

• The Russian title 'Мор. Утопия' is a pun on Thomas More's Utopia and the Russian word for 'plague' (мор).
• The Devotress/Changeling route is available from the start in the original 2005 release; the requirement to unlock it by completing the game with the Bachelor or Haruspex was introduced in the 2015 remastered version.
