According to mythology, Kambagiri Swami has seven brothers, including Lord Venkateswara Swami (also known as Balaji, Tirumala Vasa, Srinivasa), Lakshmi Narasimha Swamy, Pedda Maddileti Swami, and Chinna Maddileti Swami. The name Kambagiri Swami is described as an archaic poetic way of referring to Lord Narasimha Swami. He has two consorts, whose names are rooted in local tradition rather than mainstream Hindu canon.

The Kambagiri Swamy temple holds significance as a local center of worship for Lord Vishnu in the Nandyal district. Devotees believe that prayers and desires are fulfilled in a short span. The temple faces a severe water problem in summer, and the region is typically very hot. An annual temple fair, the Tirunnalla, occurs on the full moon day of Chatra (15 days after Ugadi), drawing lakhs of devotees, with special government buses arranged for transport. In May or thereabouts, the temple priests celebrate the celestial wedding of Goddesses Adi Lakshmi and Siddi Lakshmi with Lord Kambagiri Lakshmi Narasimha Swamy at Perusomula village. In January 2024, the temple was renovated and developed along with three other local temples at a combined cost of Rs.30 crore.
