Francis was drafted fourth overall in the 1981 NHL entry draft. He served as captain of the Whalers from 1985 to 1990 before being traded to the Penguins. He won consecutive Stanley Cups in 1991 and 1992, won the Frank J. Selke Trophy in 1995, and the Lady Byng Memorial Trophy in 1995, 1998, and 2002. He signed with the Hurricanes in 1998 and became captain in 1999, winning the King Clancy Memorial Trophy in 2002. That same year he became the fifth player in NHL history with both 500 goals and 1,000 assists. He was traded to the Maple Leafs in 2004 and retired after the 2004–05 lockout. Carolina retired his number 10 jersey in 2006.

Francis retired second in career assists (1,249), fourth in points (1,798), third in games played (1,731), and 19th in goals (549). He was named one of the 100 Greatest NHL Players in 2017. In management, he served as director of player development for Carolina in 2006, director of hockey operations in 2011, minority owner in 2012, general manager in 2014, and president of hockey operations briefly in 2018 before being fired. He became the first general manager of the Seattle Kraken in 2019, was promoted to president of hockey operations in 2025, and stepped down at the conclusion of the 2025–26 season. He was hired by the Penguins in 2026.
